    • a copy of the student’s birth certificate.
  3. Passport and Visa
    • a copy of the pages with the holder’s name and details, photo and visa (if applicable).
  4. National Identity Card (Malaysians only)
    • a copy of the IC
  5. Vaccination Record
    • students vaccination record.
  6. School Reports and Testimonials
    • a copy of the student’s official leaving certificate and any testimonial regarding the student’s character and abilities from the current school’s Principal plus 2 years’ past academic records.
    • Any specialist reports that may relate to your child’s learning e.g. Educational psychologist, Occupational therapist or Speech therapist.
  7. Application Fee
    • the non-refundable application fee (refer to schedule of fees)

Section 7 : Parent / Guardian Declaration

In making this application I/We undertake and agree:
  1. To pay a non-refundable Application Fee when submitting this form.
  2. To provide a copy of the student’s most recent school academic and behaviour reports at the time of application.
  3. The student completes the assessment tests if applicable.
  4. That should the student be offered a place at MAHSA International School he/she will participate in all normal educational activities including physical education and sports activities, scientific work, project work, enrichment or remedial programmes, swimming lessons, educational visits and outings.
  5. That the student undertakes to wear the school uniform.
  6. That the school reserves the right to discipline, suspend or dismiss any student whose lack of academic progress or whose behaviour is considered by The Principal to be unacceptable or an embarrassment to the school. When any student is suspended or dismissed (expelled), the school will retain, by way of liquidated damages and the parent shall forfeit all entitlement to, any fees paid with respect to the student.
  7. To inform the school of any changes of address, contact details or change in personal details from the month of entry for the student.
  8. To abide by any changes made to school policy, regulations, fees and schedule by the school.
  9. To pay all fees before the commencement of the school term (of which there are three per academic year). The school reserves the right to impose a late payment charge of 5% per annum on all outstanding fees which have not been paid by the 10th day of the beginning of the school term. The school reserves the right to refuse entry to any student and to terminate this contract if fees remain unpaid after the 10th day of any school term.
  10. That the school reserves the sole discretion to terminate the contract with the parent/guardian at any time if such action is considered to be in the best interest of the school.
  11. To give, in writing, on or before the first day of term, notice of a student leaving the school at the end of the term. Failure to do so will result in forfeiture of the deposit.
  1. I/We hereby indemnify MAHSA International School, its officers and employees against any and all claims from any injury to the student while participating in any school activities or while on school property or while travelling to or from school premises.
  2. I/We understand and agree that in the event of an emergency, the school will make every effort to contact the parents or guardian. However, if this is not possible or is unsuccessful, the student will be taken to either his/her family doctor (if known) or to a suitable hospital approved by the school, for treatment. The parent/guardian will reimburse the school for any expenses incurred.
